Business processes that have been mapped and analysed for more than three years will be sub-optimal. Productivity will be lower than it should.
It is also highly likely that the processes will carry a higher level of risk of failure than the organisation accepts as its normal risk profile.
How do we know this? Many years of completing safety incident investigations showed that processes left unreviewed drift away from intended practice.
People change roles, process documentation gets lost, inductions are not carried out, new equipment is brought in with an informal approach to its place in existing processes and new processes are put in place with unintended consequences for existing processes.
